What are Community Support Programs?


Community Support Programs or CSPs are community-based treatment programs designed to provide mental health treatment and rehabilitation to members of the community living with severe and persistent mental illnesses (Bipolar, Schizophrenia, and other psychotic disorders.)


The treatment provided is recovery-focused, person-centered and community-based. Being community based allows participants to receive services in the community and in their natural environment rather than in a typical office setting. Treatment is individualized based on each person’s needs and goals.

The ultimate goal of CSP is that participants will spend less time in institutional settings, and will spend more time living life in the community, working towards recovery, and meeting their life goals.

What type of treatment is provided in a CSP?


The type of services a person receives is based upon an assessment which occurs in the first 30 days following program enrollment. We also ask what participants are hoping to gain from their involvement in the program. The following list is an example of the services provided:


• Psychiatry
• Psychotherapy
• Medication management and monitoring
• Psychosocial rehabilitation
• Social opportunities
• Assistance with activities of daily living
• Benefit/case management and connection to resources
• Vocational Rehabilitation


In addition to these services, participants are involved in a treatment plan meeting every six months to identify their goals and to incorporate those goals into their treatment.

 

Who works in a CSP? What does treatment look like?


Community Support Programs are staffed by a multidisciplinary team of professionals. This team works together to provide person-centered services to individuals who are diagnosed with severe and persistent mental illness. Each participant is assigned a primary case manager, but may also have regular contact with several different staff. Using a team model allows the participant to receive high quality care that is consistent, uniform and structured to meet their needs.


How do I pay for CSP services?


Since CSP is a Medicaid-funded program, consumers typically do not have a co-pay. You can also choose to pay for your services out of pocket if you do not have Medicaid. (Please bring your insurance cards to your assessment.) You can ask for Rock County’s Payment Policy for more information about self-pay costs.
